JOB TITLE:    Community Support Services Worker

SUPERVISOR: Community Support Manager

LOCATION:  Blair County, Cambria County, Somerset County

RESPONSIBILITIES: To provide routine personal care services to assigned clients in their home; report all accidents/incidents and changes in clients condition observed on shift.

QUALIFICATIONS: High School diploma or equivalent; ability to lift, push, pull and move a minimum of 50 pounds.
