The senior living community of The Lutheran Home at Johnstown has been a part of the Westmont community and Johnstown since 1960. New to the community are bright, spacious apartments at The Lutheran Home at Johnstown that overlook Westmont neighborhoods. Amenities include:

Personal Care

The Lutheran Home at Johnstown’s personal care services provide independent seniors with the comfort, safety, security and companionship that many seek as they age.

Residents choose the living arrangement that best meets their needs and lifestyles with living options on two floors. Private rooms are furnished, but residents are certainly encouraged to bring their own belongings to personalize their rooms.

Our personal care residents are invited to make use of our spacious community areas. These areas provide the perfect environment to entertain old friends and make new ones.

The Lutheran Home at Johnstown is licensed by the Department of Human Services to provide personal care services.

Personal Care Services and Amenities

Personal care plans are individualized and coordinated with caregivers and the resident’s physician. Staff are available 24 hours a day. Services include:

Assistance with personal routines such as grooming, dressing, medication and dietary management
Dining assistance and nutrition
Private rooms with private baths

Health Care

Skilled nursing care at The Lutheran Home at Johnstown provides around-the-clock nursing supervision for long-term residents and short-term rehabilitation residents in a warm and supportive environment. The Lutheran Home at Johnstown is licensed by the Department of Health to accommodate 63 residents.

Skilled nursing services are directed by the resident’s personalized care plan, which is coordinated with caregivers and a physician. As needed, skilled nursing services include:

All skilled nursing residents are screened upon admission for physical, occupational or speech therapy needs. If services are not needed at the time of admission, residents are screened quarterly for any decline that could be improved through therapy. Our restorative nursing services and specially trained nursing staff enable residents to maintain the skills and strength they gained in therapy.

The Lutheran Home at Johnstown offers an excellent multi-leveled therapy program that can accommodate a person’s recovery process. Our therapy program is perfect for aging adults.

The rehabilitation program is committed to returning patients to their own homes as soon as it is safe to do so.

Rehabilitation Services

Rehabilitation services are individualized and delivered by full-time occupational, physical and speech therapists. Short-term stays offer accommodations close to the therapy room, and other services include:

Personal assistance, as needed
Medication management
Individual discharge plans, including in-home evaluations
Variety of therapeutic modalities to treat residents, including moist heat, electrical stimulation, ultrasound and paraffin treatments

Short-Term Rehabilitation

Short-term rehabilitation is provided to residents who may have been recently hospitalized or are recovering from medical conditions, including hip or knee replacements, hip or pelvis fractures, back surgery, falls, strokes and other injuries or surgeries.

Therapists treat each resident one-on-one. Combining occupational, physical and speech therapy, they coordinate the most efficient recovery possible to get residents back into their own homes. Our therapists then conduct in-home evaluations and, if necessary, order equipment to make the home safe and accessible, such as a raised toilet seat or grip bar for the shower.

Short-term rehabilitation residents can enjoy the benefits of restorative nursing. Therapists train the nursing staff to maintain and develop the residents’ skills and strength that they gained in therapy.

Outpatient Rehabilitation

When additional therapy is required, The Lutheran Home at Johnstown offers outpatient therapy. This therapy allows former patients and residents to continue or have therapy with the team of therapists they know and trust, and who are already familiar with residents’ therapy needs and goals.
